Available to clients who meet state eligibility requirements, Targeted Case Management (TCM) includes a comprehensive assessment of the client to determine the need for medical, educational, social or other services; the development of a comprehensive care plan based on the needs identified in the assessment; making referrals and related activities to help the client obtain needed services; monitoring and follow-up activities which may include contact with other service providers to ensure the effective implementation of the care plan; and modifying the plan as needed.
