How do I make an appointment?
A: When you call our Portsmouth office at 431-6703 or our Exeter office at 772-2710, Monday-Friday 8:00am to 5:00pm, you will be connected with the Admissions/Intake Department. When you speak with an Admissions/Intake Representative, they will gather a very brief history from you and schedule you with a therapist for an intake assessment.

Q: Will I continue to see the therapist who did my initial assessment or will I see a different therapist for on-going treatment?
A: As much as possible we work to make a good match between consumer and therapist before the first appointment. Sometimes, due to unforeseen circumstances either the therapist and/or consumer discover that the match isn’t quite right and a change is necessary. We will explain fully to the consumer the reason for the change and handle all changes carefully and sensitively.

Q: What if I don’t feel like I’ve connected with my therapist or psychiatrist and wish to change to another provider at the Center?
A: We understand that there are times when a consumer and their provider do not connect. It may be a personality issue or style of treatment that prevents a good, solid therapeutic relationship from forming. We honor consumer requests for change and encourage consumers to discuss this with their current provider to reach a resolution.

Q: How much does treatment cost, and do you take insurance?
A: We accept most major insurance plans. We offer a sliding fee scale for consumers residing in Eastern Rockingham County who meet the necessary criteria. Charges vary according to the services provided and length of appointment. Our Admissions/ Intake Representative will explain in detail the cost of treatment when you call or you can view our published rates.

Q: How often will I see my therapist?
A: Each individual’s needs are unique. During your intake (initial assessment), your therapist will discuss what form of treatment, i.e. individual therapy, family therapy, medication management, etc. you would most benefit from and how often the sessions will be scheduled.

Q: I would like a copy of my medical record. How do I obtain a copy?
A: Previous and current consumers must sign a Release of Information form in order to request copies of their own records or those of minor children. Release forms can be mailed or faxed, or they can be obtained during usual business hours at the reception desks of both the Portsmouth and Exeter offices. The first 25 pages are provided to the consumer at no charge, with a charge of .25 cents per page thereafter. Processing time for all requests is approximately 10 business days. Please keep in mind that adult records are retained for 10 years following termination of services, and children’s records to age 41. Parent or guardian must authorize release of records of children under the age of 18. Once the consumer has reached age 18, he or she must authorize release of his or her own records. Please contact the Medical Records Department to request a release form.

Q: How can I have a copy of my medical record sent to another doctor or health care provider?
A: At the consumer’s request and with a signed release as above, copies can be sent to other health care providers at no charge. Faxing of records is done only in emergency situations.

Q: How can I have copy of my medical record sent to a lawyer or other non-health care provider?
A: At the request of the consumer or third party and with a signed release, copies can be sent at a charge of .50 cents per page, as allowed by NH State Law.

Q: I’m really worried about a friend who has been very sad and talks about not enjoying life anymore. What can I do to help?
A: There are a few things you can do to help. Most importantly, if you feel your friend may harm himself or herself call 911. If you don’t believe they are in immediate danger, you can let them know that you’ve noticed a difference in them, that they seem sad and you are worried. You can give them our phone number and gently suggest that they call and speak with someone.
